Job Title: Home Health Aide – Direct Care Worker

Reports To: Director of Operations

Summary: Provides non-medical/unskilled care and companion services to adults.

Duties and Responsibilities include the following.  Other duties may be assigned.

  1. Assists clients with performing personal care activities such as bathing, shaving, toileting, and dressing.*
  2. Performs light housing-keeping duties such as washing dishes, doing laundry, changing bed linen, sweeping, mopping, or vacuuming floors. *
  3. Accompanies clients and/or arranges transportation, doctor visits, and grocery shopping. *
  4. Provides medication reminders.*
  5. Prepares meals.*
  6. Assists clients with daily tasks like walking and/or transferring from the bed to the chair.*
  7. Observes the client and reports any changes in the client’s condition or other concerns to management. *
  8. Records tasks performed in the client logbook and/or in EVV system notes. *
  9. Attends staff meetings and training as directed. *

Language Ability:

Ability to read and comprehend simple instructions, short correspondence, and memos. Ability to write simple correspondence. Ability to effectively present information in one-on-one and small group situations to customers, clients, and other employees of the organization.

Math Ability:

Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.

Reasoning Ability:

Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.

Computer Skills:

To perform this job successfully, an individual should have knowledge of Internet Software and Electronic Visit Verification (EVV) Systems (timekeeping and recording tasks).

Education/Experience:

High school diploma or general education degree (GED).  Home Health Aide or CNA Certification preferred, and one to two years related experience and/or training; or equivalent combination of education and experience preferred.

Specialized Training:

  • Required to complete State required trainings on an annual basis. Trainings may include: Reporting Critical Incidents, Reporting Abuse and Neglect, Universal Precautions, and Infectious Disease Control.

Certificates and Licenses:

  • First Aid and CPR certifications are generally preferred. May be required for some work assignments.
  • Child Abuse Clearance and Mandatory Reporter Training may be required when children are

Knowledge, Skills, Abilities and Other Requirements:

  • Must be able to successfully pass a competency exam.
  • Must be able to successfully pass a criminal background check and health screening.
  • Must be able to demonstrate empathy, patience, and compassion.
  • Excellent communication and interpersonal skills are required.
  • Must comply with all applicable vaccination mandates and health screening requirements.
