Job Description
Seeking an experienced full-time Foster Care Case Manager in the Philadelphia, PA area.
Responsibilities include:
- Provide support: Offer emotional support and counseling to children and families
- Develop care plans: Create individualized care plans for foster children
- Coordinate services: Link families to resources and services that improve quality of life
- Conduct home visits: Monitor the child’s progress and well-being including regular safety check visits
- Complete documentation: Complete mandated reports and documentation & maintain visit notes
- Provide guidance: Provide guidance and direction to foster and adoptive parents
- Oversee homes: Oversee and ensure compliance of assigned foster and adoptive homes
- Help develop life skills: Help foster children develop life skills that they need in life outside of foster care
Qualifications:
- Bachelor’s degree in Social Work or related field
- At least 2 years of experience as a Social Worker
- Preferred locations: Delaware, Chester, Montgomery & Philadelphia Counties
